Registration Fee:
$180 per player (individual registration) – includes uniform
$170 per player (team entry) – no uniform

*Minimum deposit of 8 players required to secure team entry
 
Registration Drop-Off:
Where: Metro Golf Dome, 125 Milner Avenue, Scarborough, Ontario 
When: Every Monday, Saturday & Sunday beginning March 19, 2012 (8pm - 10pm)

Payments Accepted: Cash, Cheque (payable to “JS Sports”) & Email Money Transfer

NO REFUNDS will be issued on registration fees

2012 Indoor Soccer Leagues, Dates & Times

Men's League - Open Division:
Wednesday between 8pm to 12am

Men’s Over 35 League:
Tuesday between 8pm to 12am

Co-Ed League:
Tuesday League between 8pm to 12am    
Thursday League between 8pm to 12am

Game Format:

• 15 players (maximum) per team (inclusive of 4 women minimum per Co-Ed team)
  
• 6 vs 6 (inclusive of goalie)
• 2 x 25 minute halves with a 5 minute break between each half
• 10 minute grace period will be granted, upon expiration of the grace period, team will go into default and a fine of $100 may be enforced
• 1st Yellow card = $0 fine, 2nd Yellow card = $25 fine and 1 game suspension
• 1st Red card = $25 fine and 2 game suspension, 2nd Red Card =  $100 fine and ban from JS Sports league (no refund on registration fees)
  
• All players must wear appropriate attire which includes:
  • team jersey
  • socks
  • shin guards
  • cleats or turf shoes
  • All jewellery must be removed
• $250 Refundable Bond due and payable by each team

Cup Game:

• Top teams will advance to playoffs (determined by standing in regular season)
• Single elimination will determine advancement to final championship game
• Overtime = 2 x 10 minutes halves (golden goal), otherwise, shootout (3 players)
• All other rules and regulations will follow FIFA standards
